Service Description: <DIV STYLE="text-align:Left;"><DIV><DIV><P><SPAN STYLE="font-weight:bold;">Hospitals</SPAN><SPAN>: Institutions that are engaged in providing, by or under the supervision of physicians, to inpatients diagnostic services and therapeutic services for medical diagnosis, treatment, and care of injured, disabled, or sick persons, or rehabilitation services for the rehabilitation of injured, disabled, or sick persons. </SPAN></P><P><SPAN STYLE="font-weight:bold;">Medical Facilities</SPAN><SPAN>: The North Carolina State Center for Health Statistics in cooperation with the Division of Facility Services in the Department of Health and Human Services, developed the GIS data set, Medical Facility Point Locations, to identify locations of health and medical facilities for the entire State of North Carolina. Point locations were geocoded from the facilities database provided by the Division of Facility Services. </SPAN></P><P><SPAN STYLE="font-weight:bold;">Public Health Departments</SPAN><SPAN>: Governmental public health departments are responsible for creating and maintaining conditions that keep people healthy. A local health department may be locally governed, part of a region or district, be an office or an administrative unit of the state health department, or a hybrid of these. Furthermore, each community has a unique "public health system" comprising individuals and public and private entities that are engaged in activities that affect the public's health. </SPAN></P><P><SPAN STYLE="font-weight:bold;">Nursing Homes</SPAN><SPAN>: A nursing home traditionally offers 24-hour (skilled) nursing to the elderly or to disabled patients having a variety of medical conditions who require personal care services above that of an assisted living but do not require hospitalization.</SPAN></P><P><SPAN /></P><P><SPAN /></P></DIV></DIV></DIV>
